About the role

Our Community Teams support adults facing a range of complex challenges associated with substance use. Based within local community hubs, you'll play a vital role in helping people achieve their recovery goals through: - Case management and care coordination - One-to-one psychosocial interventions - Harm reduction advice and support - Monitoring and supporting prescribed treatment alongside clinical colleagues - Working in partnership with a range of agencies to develop holistic recovery plans that promote long-term wellbeing, independence, and resilience Every day brings the opportunity to make a meaningful difference in someone's life.
