Substance Misuse Case Manager - Community Team

About the role

Our Community Teams support adults facing a range of complex challenges associated with substance use. Based within local community hubs, you'll play a vital role in helping people achieve their recovery goals through: - Case management and care coordination - One-to-one psychosocial interventions - Harm reduction advice and support - Monitoring and supporting prescribed treatment alongside clinical colleagues - Working in partnership with a range of agencies to develop holistic recovery plans that promote long-term wellbeing, independence, and resilience Every day brings the opportunity to make a meaningful difference in someone's life. About You We're looking for enthusiastic, compassionate individuals who are committed to supporting people to create positive change. You will hold one of the following: - A relevant degree such as Psychology, Sociology, Social Work, or a related discipline; or - A Diploma in Health and Social Care (or equivalent qualification); - A Level 3 qualification in Tackling Substance Misuse (or equivalent) Most importantly, you'll share our commitment to delivering high-quality, recovery-focused support that places individuals at the heart of everything we do.
