Your Role as a Support Coordinator As a Support Coordinator, you'll work directly with Medicaid participants in self-directed care programs. Your role is to educate, guide, and support them in managing their care plans, hiring direct care providers, and ensuring their home environment is safe. You'll be out in the field meeting participants in their homes for at least 60% of your work hours, ensuring they have the resources they need to remain independent. The rest of your time will be spent completing documentation and connecting with team members from your home office.
