In this role, you'll conduct comprehensive medical evaluations, develop and manage individualized treatment plans, prescribe and monitor medications, and coordinate closely with physicians, caregivers, and interdisciplinary team members. Working within a collaborative practice framework, you'll serve as a key clinical decision-maker while helping patients and families better understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations. Location Office Location: Grand Rapids, MI Main Service Area: Grand Rapids and surrounding areas This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes Hours Monday to Friday, 8 a.m. to 5 p.m. Required Qualification Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant Current State License as a Nurse Practitioner or Physician Assistant Reliable transportation, valid driver's license, and valid automobile insurance coverage

Requirements

  • Minimum 2 years clinical experience in a post-acute or as a Nurse Practitioner or Physician Assistant
  • Adult, Family, or Gerontology certification
  • Experience with geriatric patients
  • Essential Skills and Competencies
  • Nursing skills as defined by accepted nursing standards
  • Demonstrates ability to assess and respond to the needs of patients, families and caregivers in post-acute care settings
  • Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
  • Ability to handle multiple priorities, documentation requirements and deadlines
