Job Title: Social Worker - Locality Team Location: Tower Hamlets Pay Rate: £ 32.53 per hour Job Description: Liquid Personnel is recruiting a skilled and proactive Social Worker on behalf of its client in Tower Hamlets. Based within the Locality Team, you will support adults with care and support needs across a defined area. The role focuses on promoting independence and delivering timely, strengths-based interventions through a person-centred approach. What will your responsibilities be? Undertake Care Act assessments, reviews, and safeguarding enquiries. Develop and implement care and support plans in partnership with individuals and carers. Work collaboratively with health, housing, and community partners to deliver integrated care. Respond to crisis situations and manage risk effectively. Promote early intervention and preventative strategies. Maintain accurate and timely case records using appropriate systems.
