Job Description: Under the leadership of the Diabetes Services Program Manager, the Diabetes Services Educator is responsible for providing patient education to patients in the outpatient and community settings. The Educator is an active member of the health care team and participates in multi-disciplinary rounds. The Educator must be able to interact empathetically and compassionately with a variety of patients and their families concerning acute and chronic medical needs. The Educator must be able to articulate the medical science related to diabetes and translate their knowledge into layman's terms in one-to-one counseling sessions, group classes, public speaking engagements, while working with print, audio, and video media. The Diabetes Educator possesses a broad scope of knowledge and skills to provide person-centered education to facilitate patient self-care management.
