As a Care Navigator you will: - Patient Assessment: Conduct comprehensive assessments of patients' behavioral health needs upon admission, including mental health status, psychosocial factors, and discharge planning requirements. - Care Coordination: Develop, implement, and monitor individualized treatment plans in collaboration with patients, families, and healthcare providers to ensure continuity of care throughout the inpatient stay. - Multidisciplinary Collaboration: Work closely with psychiatrists, psychologists, social workers, nursing staff, and other members of the care team to facilitate effective communication and collaboration regarding patient care - Documentation: Maintain accurate and timely documentation of assessments, care plans, and progress notes in compliance with regulatory standards and organizational policies. - Discharge Planning: Initiate and coordinate discharge planning early in the hospitalization process, connecting patients to appropriate community resources and outpatient services post-discharge. - Patient Advocacy: Serve as a patient advocate, addressing concerns and ensuring that patients and families are informed and involved in treatment decisions. - Quality Improvement: Participate in quality improvement initiatives and contribute to program development aimed at enhancing the delivery of inpatient behavioral health services. - Crisis Intervention: Respond effectively to crisis situations, providing support and stabilization for patients in distress. - Education and Support: Provide education and resources to patients and families regarding behavioral health conditions, treatment options, and coping strategies. Qualifications include: - Master's degree in social work, counseling, psychology, or a related field required. - Minimum of 2 years of experience in behavioral health care management, case management, or a related field, with a strong understanding of inpatient behavioral health settings. - Current licensure in a relevant field (e.g., LMSW, LPC) required - Certification in care management or behavioral health (e.g., CCM, PMH-C) is a plus.
