Join us and discover a supportive environment where your career can truly thrive. ๐ฑ ๐๏ธ Key Responsibilities In this role, you will: Provide nursing care based on your licensure/certification and scope of practice Maintain up-to-date clinical records for each patient, meeting documentation deadlines Continuously evaluate the effectiveness of nursing care for patients and families Consult with physicians on changes to patient care plans and report updates to the nurse supervisor Support FFHC's mission by mentoring/coaching new employees during patient-specific orientations Educate patients and caregivers on care needs and equipment Participate in FFHC's Quality Assurance and Performance Improvement (QAPI) program as requested Continually strive to improve your nursing skills and knowledge Attend/complete all mandatory trainings and in-services Submit required personnel documentation in a timely manner Adhere to FFHC compliance and ethics standards Maintain patient and family confidentiality Demonstrate professional, supportive, and responsive communication Perform other duties as assigned ๐ Role Qualifications Required: ๐ LPN/LVN Nursing Degree ๐ชช Unencumbered LPN/LVN license in the state of employment or multi-state license through the Enhanced Nurse Licensure Compact (where recognized) ๐ 1+ year of nursing experience ๐ Valid driver's license with proof of insurance (in applicable states) โ Clear background screening ๐ Working knowledge of Nurse Practice Acts โค๏ธ Valid and current CPR card/certificate with in-person, instructor-led hands-on skills check ๐งช Clear Hep B screening (in applicable states) ๐ฉบ Clear TB screening (in applicable states) Preferred: Pediatric and/or home care experience Comfort working independently in the home setting State-Specific Requirements (Examples): Florida: Standard requirements as above Illinois: Driver's license with proof of insurance not required (staff are prohibited from driving patients) Hep B screening completed during onboarding (not a prerequisite for hire) Pennsylvania: Additional background checks: Child Abuse and FBI TB screening with 2-step PPD or 1-step PPD with proof of 2-step within the last year Texas: N/A (standard requirements apply) ๐ง Key Competencies Ability to read and communicate in English , both verbally and in writing Strong interpersonal skills and a supportive, team-oriented mindset ๐ก Working Conditions & Equipment Work is performed in the patient's home - not substantially exposed to adverse environmental conditions Training may occur in an office setting Must be able to lift up to 35 lbs without an assisted lifting device Frequent use of office and clinical equipment (telephone, computer, printer, etc.) ๐ Important Notes Job Descriptio
