About the role

As a Home Health Occupational Therapist, you'll work one-on-one with patients in their homes - teaching new skills, retraining lost abilities, and creating innovative solutions that enable them to regain independence in their daily activities. Responsibilities

  • Teach new skills or retrain patients in once-familiar daily activities lost due to illness or injury
  • Provide therapeutic activities designed to restore function and self-care abilities
  • Demonstrate and teach alternate techniques for completing activities of daily living
  • Focus on improving fine motor skills, coordination, and cognitive abilities for functional tasks
  • Work toward patient and caregiver independence as the ultimate goal
  • Assess and Create Individualized Plans
  • Conduct comprehensive initial and ongoing assessments of functional level using appropriate tests
  • Develop and revise treatment plans in consultation with physicians and care team members
  • Set realistic, measurable goals that address what matters most to each patient
  • Evaluate home environments and identify modifications to support independence
  • Assess ongoing care and equipment needs throughout the episode
  • Provide Adaptive Solutions
  • Order and/or fabricate orthotic, adaptive, and assistive devices tailored to patient needs
  • Train patients and families in the use of adaptive equipment and assistive technology
  • Recommend home modifications and safety equipment to support independence
  • Teach proper transfer techniques and positioning strategies
  • Design creative solutions to overcome environmental barriers in the home
  • Educate and Empower
  • Instruct patients, families, and caregivers in exercise programs to increase strength and control
  • Teach energy conservation and work simplification techniques
  • Provide education on disease prevention and injury management
  • Counsel families on home rehabilitation procedures and ongoing care needs
  • Supervise and instruct occupational therapy assistants in patient-specific care plans
  • Collaborate with the Care Team
  • Coordinate with physicians, nurses, PTs, MSWs, and other healthcare professionals
  • Attend and initiate patient care conferences for complex cases
  • Make referrals to other disciplines when indicated by patient needs
  • Communicate patient progress, reactions to treatment, and condition changes
  • Participate in quality improvement initiatives and QAPI activities
  • Maintain Clinical Excellence
  • Complete accurate, timely documentation according to agency policies
  • Monitor and report changes in patient condition to physicians and care managers
  • Stay current with occupational therapy trends and evidence-based practices
  • Participate in discharge planning to ensure smooth transitions
  • Evaluate outcomes of care and adjust treatment approaches accordingly

Requirements

  • Required Qualifications
  • Graduate of an accredited occupational therapy program (ACOTE accredited)
  • Current state license as an Occupational Therapist
  • Minimum 1 year of experience as a licensed Occupational Therapist
  • Reliable transportation with valid automobile insurance
  • Willingness to travel daily for home visits within service area
  • Home health experience
  • Experience with adaptive
