We are seeking two Mental Health Case Managers to join our Community and Mission team across the NENW. This role delivers long-term, recovery-oriented case management to adults living in the community with persistent mental health conditions. You will support clients to build capacity, access community connections, and work towards their personal goals while collaborating with internal teams and external stakeholders across the New England Northwest region. Provide person-centered, strengths-based mental health case management Develop, implement, and review individual client plans and goals Support community access, social participation, and daily living skills Provide advocacy, practical assistance, and capacity building Maintain an active caseload in line with funding and program requirements Build strong relationships with community services and stakeholders Complete accurate documentation and contribute to program quality and safety Travel across the region as required
