Liquid Personnel is recruiting a Social Worker to join its client's Generic Team, based in Wirral. This is a hybrid role offering £33.97 per hour. What will your responsibilities be? In this role, you will: Conduct assessments - Carry out strength‑based assessments to determine adults' needs, risks, and eligibility for support in line with statutory frameworks Develop care plans - Formulate, implement, and review person‑centred care and support plans that promote independence, wellbeing, and safeguarding Multi‑agency collaboration - Work closely with health professionals, community services, and partner agencies to coordinate safe, timely interventions and ensure continuity of care
