• Please bring the following if you are currently certified:
• Valid New York State HHA or PCA Certification
• Proof of U.S. Work Authorization (2 forms of ID, such as a Social Security Card and Valid Photo ID)
• Preferred Documents
• If available, please bring:
• Current Physical Examination (within the last year)
• Two-Step PPD or QuantiFERON TB Test
• Measles, Mumps, and Rubella (MMR) Vaccination Records
• Health Assessment/Drug Screening Documentation
• Proof of Flu Vaccination (optional)
• Two Professional Reference Letters
