Anticipated End Date: 2026-07-13 Position Title: Behavioral Health Case Manager I - New York Medicaid Job Description: Behavioral Health Case Manager I - New York Medicaid Location: The ideal candidate must reside within the state of New York to be considered for this role. Field : This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement. Please note that per our policy on hybrid/virtual work, candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment, unless accommodation is granted as required by law. Schedule: Monday -Friday, 8:30AM - 5PM EST. No weekends or holidays. The Behavioral Health Case Manager I will be responsible for performing case management telephonically and/or by home visits within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs. How you will make an impact: Us es appropriate screening criteria knowledge and clinical judgment to assess member needs. Conducts assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment. Monitors and evaluates effectiveness of care plan and modifies plan as needed. Supports member access to appropriate quality and cost effective care. Coordinates with internal and external resources to meet identified needs of the members and collaborates with providers. Minimum Requirements: Requires MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing, and minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background. Current active unrestricted license such as LCSW (as applicable by state law and scope of practice) LMHC, LMSW (as allowed by applicable state laws) in the state of New York is required. Preferred Skills, Capabilities and Experiences: Experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders preferred. For candidates working in person or virtually in the below locations, the salary* range for this specific position is $67,452 to $96,360. Location: New York.
