Liquid Personnel is recruiting an Adult Social Worker to join its client's Hospital team, based in Manchester.
What will your responsibilities be?
Managing hospital discharges - completing timely Care Act assessments, coordinating safe discharge plans and ensuring appropriate community support is in place.
Liaising with reablement and MDTs - working closely with therapy teams, ward staff and external providers to progress discharge pathways and reduce delays.
Undertaking hospital‑based social work tasks - addressing safeguarding concerns, assessing risk, updating support plans and supporting complex cases within a fast‑paced acute setting.
