  • Principal Duties and Responsibilities
  • Provides assistance with receiving all incoming referrals and scheduling assessment visits
  • Schedules and completes assigned client/family assessments to determine appropriateness for admission to Transitions program
  • Identifies the needs of client/family and sets goals to meet those needs. Implements and coordinates plan of services.
  • Facilitates on-going assessments/re-evaluations of client/family
  • Appropriately refers clients/families to services in the community
  • Documents visits and phone calls in data base to ensure accurate client/family records.

  • $58,488.99 - $93,582.39 Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.

Under the direction of the Transitions Manager, assesses patients' appropriateness for admission to the program. Completes in home assessments for admissions to Transitions and provides follow-up calls and visits as needed to Transitions clients. Identifies full range of services to meet client needs. Education Master's Degree in Social Work required
