Union: Non-Union Number of vacancies: 1 New or Replacement Position: New Site: Toronto Western Hospital Department: Krembil Brain Institute Reports to: Manager Salary Range: $45.85 - $57.31 per hour Hours: 37.5 Hours per Week Shifts: Monday to Friday Status: Temporary Full-Time (12 Months Contract with possibility of extension) Closing Date: July 9, 2026 Position Summary As an integral member of the Epilepsy Team, the responsibilities of the EPILEPSY CLINICAL COORDINATOR , include but is not limited to: Utilizing and applying current specialized knowledge and expertise on recent advances in epilepsy care practice to facilitate the effective management of in/out-patient care and continuity in patient care management Liaison from Epilepsy Monitoring Unit (EMU) journey up until resective surgery or decision made to not continue with surgery, involving the coordination, implementation, and evaluation of the treatment plan by participating as a consultant, educator Supporting a patient population that often presents with significant complexity, including co-morbid cognitive and psychiatric conditions, requiring thoughtful coordination, clear communication, and sensitivity to unique patient needs. Collaborating in the establishment and development of policies/procedures which comply with best practice guidelines Leading the creation of a culture that acknowledges and acts on the benefits of appropriate care in all patients in the Epilepsy program and participating in academic and research projects Performing teaching/education responsibility Participating actively in Quality Improvement initiatives Performing administrative responsibilities Performing cross-functional and/or other duties consistent with the job classification, as assigned or requested. Duties Provide clinical leadership and coordination across the epilepsy care pathway, supporting both inpatient and outpatient care delivery Serve as the primary point of contact for patients, guiding patients from EMU waitlist through post-admission follow-up and surgical decision-making Lead EMU waitlist prioritization, including patient risk stratification, readiness assessment, and coordination with clinical teams to optimize admissions and reduce delays Identify and escalate clinical risks, barriers, and changes in patient status in collaboration with Clinical Manager, Nurse Practitioner, and Social Worker Coordinate and triage complex patient care activities across Neurology, EMU, and Surgical teams to ensure timely, safe, and efficient care Facilitate and coordinate weekly Epilepsy Rounds, including case preparation, multidisciplinary participation, and tracking of decisions and follow-up actions Ensure continuity of care by monitoring patient progress, reinforcing care plans, and preventing gaps across the epilepsy pathway Support patient and family education by reinforcing care plans, preparing patients for EMU admission, and ensuring consistent, high-quality teaching resources and practices Act as a clinical expert resource and liaison for internal teams and external partners, promoting best practices in epilepsy care Identify trends and opportunities for improvement in care delivery and contribute to the design and implementation of process improvements Participate in quality improvement initiatives, including data tracking, audits, and evaluation of patient outcomes Contribute to policy, protocol, and education resource development in collaboration with interdisciplinary teams Maintain accurate documentation, data tracking systems, and program statistics to support clinical care and reporting requirements Participate in committees, education activities, and stakeholder engagement to advance program goals Current registration with the College of Nurses of Ontario Completion of a Bachelor's of Science degree in Nursing Completion of Basic Cardiac Life Support (B.C.L.S.) program Over 2 years practical related experience Strong organizational and time management skills Excellent interpersonal and customer service skills Excellent computer skills including Microsoft Office and EPIC applications Excellent verbal and written communication skills Knowledge and experience with epilepsy is an asset Demonstrated ability to cultivate and build relationships Strong track record of high-quality work and engagement is essential Able to adapt to a constantly changing environment that requires ongoing revaluation of priorities Demonstrated ability to use good judgment in assessing difficult situations Able to work collaboratively within a group setting Proven track record of taking initiative Why join UHN?
