Our client in Worcestershire is looking for an experienced Social Worker to join their Area Team for Adult Social Care. What will your responsibilities be? As a Social Worker, you will conduct thorough social care assessments to identify the needs of adults with physical or mental impairments, frailty, and mental health concerns related to old age. You will develop and implement personalised care plans, provide crisis intervention support, and manage a caseload effectively. Collaborating with multi-disciplinary teams, you will contribute to assessments that improve client outcomes while ensuring safeguarding measures are in place to protect vulnerable adults. You will work closely with reablement services to maximise independence following hospital discharge or within a community setting, ensuring compliance with all relevant legislation, policies, and procedures.
