Job Description Baystate Health, a nationally recognized leader in healthcare quality and safety, and home to Baystate Hospice is looking for a part-time Home Health Aide to join the team! This position provides support and assistance with personal care. Assists patient in functioning in their home setting and encourages independence when possible. Maintains safe, healthy environment for patients by completing assigned tasks on the Home Care Aide Plan of Care. Schedule: Full-time day shift 8:00am - 4:30pm - with some flexibility Weekend & holiday rotation On the road - Hampden/Hampshire county ( mileage reimbursement ) HOSPICE/HOME HEALTH DEPARTMENT DESCRIPTION: Baystate Home Health and Hospice interdisciplinary care teams consist of experienced nurses, therapists, social workers, home health aides, spiritual and bereavement counselors, and volunteers. Care teams work together with both the patient and family, to ensure quality end of life care. All care is expressly tailored to meet each patients' needs. Competitive rates with a 20% weekend differential Direct Deposit Paid Time Off: Holiday, Sick, Vacation, Personal HHA REQUIREMENTS: Home Health Aide or CNA Certification Valid Driver's License HiSet/GED or High School Diploma POSITION RESPONSIBILITIES: 1) The Home Health Aide provides emotional and physical support to terminally ill individuals and their caregivers. 2) The Home Health Aide assists with all personal care duties in a caring and professional manner, encouraging patient independence within the patient's functional limitations. 3) The Home Health Aide assists the caregiver in teaching self care to the patient to maintain and promote the independence of individuals in a safe and healthy environment. 4) The Home Health Aide performs duties indicated in the patient care plan as outlined by clinical staff and demonstrates good judgment and skill and takes appropriate initiative to insure that patient care is of optimum quality. Education: GED or HiSET (Required) Certifications: Basic Life Support - American Heart Association, Certified Home Health Aid - OtherOtherOther, Driver License - OtherOtherOther
