We are expanding with a new, one-million-square-foot, 15-floor, state-of-the-art facility on Cleveland Clinic Main Campus designed to elevate care for complex neurological conditions - including spine injuries, epileptic seizures, movement disorders, multiple sclerosis and stroke. This advanced building brings every stage of care under one roof, featuring dedicated spaces for outpatient services, surgery, stepdown, ICU and epilepsy monitoring, as well as additional operating rooms and expanded ambulatory, infusion, imaging, EMG, sleep and rehabilitation services. As a Case Management Social Worker, you will collaborate with interdisciplinary healthcare teams across the continuum of care to coordinate seamless patient transitions and deliver compassionate, patient-centered support. You will apply your expertise in crisis intervention, care coordination, psychosocial assessment, discharge planning, resource navigation, counseling, and patient advocacy to assist individuals and families from diverse backgrounds with complex medical, behavioral, and social needs. Your work will directly influence patient outcomes by empowering individuals and families to self-manage their care, access community resources, and achieve greater stability, recovery, and overall well-being. This position is neuro-specific and will be embedded directly within the nursing units of the neuro building, working closely with ICU and inpatient care teams to provide social work consult support. Candidates should possess prior social work experience, though hospital-based experience is not required; however, new graduates will not be considered for these roles. Team members will participate in an extensive 8-12 week onboarding and training program, including dedicated neuro-focused training and cross-coverage preparation alongside experienced hospital case managers. While the primary focus will be consultative social work support within ICUs and nursing units, staff will also receive training to assist with cross-coverage responsibilities as needed. A caregiver in this position works f ull- t ime| days| 8 :00am-4:30pm A caregiver who excels in this role will: Manage all aspects of discharge planning for assigned patients to ensure a timely discharge and provide appropriate connection with post-discharge care providers. Communicate with interdisciplinary team about the discharge planning status of all patients referred. Educate patients, family and physician regarding post-acute options and address freedom of choice and financial concerns. Conduct initial and ongoing psychosocial assessment and interventions and makes treatment recommendations to engage patients and families in adherence to the treatment plan by reducing or eliminating the social, psychological, financial, behavioral, and regulatory barriers to successful medical outcomes. Serve as a resource person to provide counseling and intervention related to treatment decisions and end-of-life issues. Advocate for patient and family by providing intervention in cases involving child abuse and neglect, domestic violence, elderly abuse, institutional abuse, and sexual assault. Advocate for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system and community. Promote individual, professional growth and development and meet mandatory and continuing education requirements. Support department-based goals which contribute to the success of the organization.
