About the role
Job Summary: Delivers Enhanced Home Personal Care (EHPC) services to frail elderly clients to support safe ageing-in-place within the community. The role provides responsive, short-duration personal care and emergency support services during night hours (8:00 PM to 8:00 AM). This includes supporting existing. Enhanced Home Personal Care (EHPC) clients through scheduled night interventions, urgent unscheduled care requests with a one-hour response time, as well as attending to community emergency activations. Principal Responsibilities and Duties: Delivery of Enhanced Home Personal Care services for both scheduled and unscheduled care needs Respond to unscheduled care requests within one (1) hour of receiving the call Conduct on-site assessment and provide appropriate personal care support Maintain a safe, clean and hazard-free home environment Clean immediate living environment when contaminated with urine, stool or other bodily fluids Change linen or bedsheets when soiled during the night Provide personal care support to clients Hygiene care, assisted shower or bed sponging Change clothes or diapers Assist with toileting Lift, transfer and reposition clients safely Scheduled Support Attend to scheduled care interventions as per assigned care plan (housekeeping and personal care) Medication & Monitoring Assist and remind clients with medication Monitor medication compliance Monitor vital signs (temperature, BP, pulse) Higher Care Tasks Simple wound dressing Stoma care PEG tube care Nebuliser with normal saline Hypo-count and documentation Care for clients with permanent tracheotomy Apply cold compress as instructed Document all services provided and report any significant changes in client condition Other responsibilities Provide administrative support Maintaining cleanliness of care outposts Support operational needs of the organisation as required Perform any other ad-hoc duties as assigned by Supervisor or Management. Essential Qualifications: For Local Applicants Minimum GCE N Level or equivalent and Training in at least one of the following: Workforce Skills Qualifications (WSQ) Higher Certificate or, Advanced Certificate in Healthcare Support (Nursing Care) or, ITE Approved Training Centre - Healthcare (Home Care) Programme For Foreign Applicants Completed more than 6 months Nursing Aide Course from home country and, Relevant caregiving or healthcare experience preferred Core Competencies Skills and Attributes: Able to communicate effectively with elderly clients Demonstrates strong self-management, accountability and professionalism in managing duties independently Physically fit and able to perform hands-on caregiving duties Patient, compassionate and respectful towards seniors. Observant, safety-conscious and responsive in urgent situations Adaptable, reliable and a team player with a passion for eldercare Able to cycle as this will be main mode of transport in-between visits Locations Ang Mo Kio Toa Payoh Jurong
