Through our Dementia Advisory Service, we work alongside older people and their families to navigate the challenges of dementia with confidence, compassion and practical support. We're now seeking a Dementia Care Partner to join our team , servicing clients across the Hills area and Northern region (including Northern Beaches, Lane Cove, Ryde, Hornsby, Pennant Hills, Carlingford and Castle Hill. Your Opportunity Servicing clients across Northern Beaches / Hills Area s Full Time | Monday to Friday| 38 hours per week | 18 ‑ month contract Hours: 8:30am - 4:30pm or 9:00am - 5:00pm This is a community based role supporting people living with dementia and their families through education, guidance, advocacy and tailored support. You'll work closely with clients in their homes and in community settings, helping them navigate services, understand dementia and maintain quality of life. In this role, you will: Provide person-centred support, advice and advocacy to clients living with dementia Conduct assessments and develop care plans to support individual needs Confident in presenting Dementia Education within Anglicare and the Community Deliver education and psychosocial support to families and carers Support clients to access services and navigate the aged care system Monitor client progress and adapt support as needs change Maintain accurate documentation and meet program KPIs Build strong relationships with families, clinicians and community networks
