The listed pay range or pay rate reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time). Pay Range: $63,356.80 - $82,763.20 Scheduled Weekly Hours: 40 Position Overview This role involves visits to homes, nursing facilities, assisted living facilities and hospitals, ( approx. once per week) to participants of the LIFE program. The organization offers mileage reimbursement for distances driven. The Registered Dietician PACE serves as a key member of the interdisciplinary team (IDT) at the PACE center and is responsible for assessing and monitoring the nutritional status of participants, developing individualized care plans and providing dietary education and support. This role also promotes optimal health and functional status for frail older adults through evidence-based nutrition practices, regulatory compliance and participant-centered care. ESSENTIAL FUNCTIONS Conducts initial and periodic nutritional assessments via face-to-face encounters across all care settings, including but not limited to the Center Clinic, Skilled and other Facilities, hospitals and participant homes. Develops and revises individualized nutrition care plans based on assessments and input from the IDT. Monitorsr the nutritional status of participants and make recommendations for interventions as needed. Provides nutritional counseling and education to participants, caregivers, and staff. Oversees the development and modification of therapeutic diets and menu planning in collaboration with food service staff. Conducts periodic reviews of dietary services, including kitchen inspections and meal audits. Maintains accurate and timely documentation in the electronic health record system Participates actively in IDT meetings, care planning, and case conferences. Ensures compliance with local, state, and federal regulations, including CMS and PACE-specific standards. Assists in quality improvement initiatives related to nutrition and food services. Performs other job duties as required. MINIMUM REQUIREMENTS Education: Bachelor's Degree in Dietetics, Nutrition, or related field required. Current Registered Dietitian credential through the Commission on Dietetic Registration (CDR). Licensed or certified in the state of NJ Valid NJ Drivers license Experience: One year experience working with a frail or elderly population preferred. Experience working with clinical charts/records. Knowledge of clinical nutrition standards, therapeutic diets, and regulatory requirements (PACE, CMS, state). Knowledge and Skills: Strong organizational, communication, and teamwork skills Mental, Behavioral and Emotional Abilities: Ability to develop strong interpersonal relationships with coworkers and participants.
