Covey Ln, Phoenix, AZ 85024 Starting Salary Range: $130,000-$135,000 Schedule: Rotating Shifts - 1 Week Monday - Friday 1pm - 9:30pm; the next week 9am - 5:30pm and rotating weekends (about 4 weekends per year) Essential job functions: Review, verify, and dispense prescription medications for long-term care residents in compliance with federal, state, and pharmacy regulations Perform clinical review of medication orders for appropriateness, dosing accuracy, drug interactions, allergies, and therapeutic duplications Exercise professional judgement in determining whether to dispense a prescription as written or make a permissible adaptation in accordance with state law, pharmacy policy, and clinical appropriateness Collaborate with prescribers, nursing staff, and facility administrators to clarify orders, recommend therapy changes, and resolve medication-related issues Ensure timely processing of new admissions, discharges, STAT orders, and medication changes to support uninterrupted patient care Oversee medication packaging processes including unit-dose and multi-dose compliance packaging Conduct and document clinical interventions, medication reconciliations, and therapy monitoring Supervise and support pharmacy technicians to ensure accurate data entry, medication fulfillment, and workflow efficiency Participate in quality assurance activities, including error reporting, root-cause analysis, and continuous process improvement initiatives Review and approve emergency kits, controlled substance documentation, and narcotic reconciliation logs Ensure adherence with USP 797, 795 guidelines for compounding Serve as manager on duty when assigned, providing leadership, decision-making, and operational oversight Support infection control, antibiotic stewardship, and immunization initiatives as applicable Dose, monitor, and manage intravenous antibiotic therapies, including renal dose adjustments, therapeutic drug monitoring (vancomycin, aminoglycosides), duration of therapy, and lab review Participate in training, onboarding, and mentoring pharmacy staff Uphold Saliba's Pharmacy standards for professionalism, patient safety, and customer service Other essential functions and duties may be assigned as required Education and/or Certifications: - Bachelor's Degree (Pharmacy) or PharmD. required - State Pharmacist License required without restrictions
