This role is responsible for achieving optimal patient outcomes through both independent practice and collaborative teamwork within the clinical setting. The Registered Nurse II will apply the Nursing Process to assess, plan, implement, and evaluate care, contributing as a trusted and integral member of the healthcare team. Patients are supported by experienced and compassionate professionals, including physicians, nurse practitioners, physician assistants, and nurses, who provide in-home medical visits. Through regularly scheduled appointments, our team builds meaningful relationships with patients, caregivers, and families-helping to enhance comfort, trust, and overall quality of life for homebound individuals. This role is split between triaging for the practice as well as home visits. Primary Care at Home | ChristianaCare Work Schedule: Monday through Fridays: 8am to 4:30pm This position would cover North Wilmington in the areas of 19810, 19809, 19703 (Claymont), and 19806 in the city of Wilmington. Duties / Responsibilities: As a Registered Nurse II , this role will provide age-appropriate, patient-centered care to pediatric through geriatric populations in both virtual and in-person settings, using strong clinical knowledge to assess and address patient needs. This role includes delivering services such as wellness visits, care management, point-of-care testing, and triaging patients, while collaborating closely with providers to support examinations, procedures, and efficient clinic operations. As a Registered Nurse II , will serve as a key liaison among patients, providers, and staff, ensuring clear communication, coordinating workflow, and responding to urgent situations. Will also support staff development through training and mentorship, participates in quality and compliance initiatives, and contributes to process improvement efforts-all while maintaining a strong focus on safety, patient education, and high-quality care delivery. Education Requirements: A graduate of a state approved or National League of Nursing accredited School of Nursing. Registered Nurse licensure or eligible for licensure in the state Delaware, Maryland or Pennsylvania & New Jersey depending on location of employment. Two years' experience as a Registered Nurse, physician office experience is preferred. Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (AND) will be considered with an agreement of completion of BSN in 3 years Basic Life Support (BLS) required ****Proof of a valid driver's license and active auto insurance is required.
