Responsibilities
- Serve as the contact point, advocate and informational resource for clients, care team, family/caregivers and community resources regarding client's service plan
- Partner with clients to monitor adherence to their service plan, evaluate effectiveness, monitor progress in a timely manner and facilitate changes as required.
- Facilitate client linkages, referrals and access to appropriate treatment, community supports and medical or specialty providers according to the service plan
- Maintain an active caseload of approximately 10-12 families, providing in person in home therapeutic sessions to address treatment plan goals

- Support the Quality/Compliance/Risk function as required.
- Performs other related duties as assigned.
Requirements
- Bachelor's level practitioner with at least 2 years of experience in social service-related field preferred.
- Must have and maintain a valid driver's license with an acceptable driving record
- Must be able to exercise independent thinking and good judgment under all circumstances.
- Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.
- Additional Information:
- Starting Base Rate: $19.23
- Bilingual rate: An additional $1.50 added to the base rate if Spanish proficient
- Schedule: Day
