Assessment Specialist -Afternoon & PM Shifts Available-LMSW or LCSW or LPC

Responsibilities

• Completes Crisis Assessment independently via in person and/or tele psych
• Documents accurately in patient chart and has the ability to update the team during shift changes
• Facilitates Admissions / assessments with Physician partners
• Performs crisis intervention
• Telephone Triage Activities
• Contributes to the organization by actively participating in process improvement initiatives and shared decision making

• TECHNICAL CAPABILITIES
• Patient Documentation (Intermediate): The ability to receive and record patient information.

Able to succinctly document clinical concerns in the patient record as well as possess mental health knowledge to refer patients to the appropriate level of care on the behavioral health continuum. . Shifts Available: 11:00AM-11:00PM & 7:00PM-7:00AM
