Liquid Personnel is seeking an experienced Occupational Therapist to join a dynamic, fast‑paced Integrated Discharge Service (IDS) within Islington Adult Social Services. You'll work as part of a multidisciplinary team of OTs, PTs and social workers supporting residents returning home safely and independently from hospital. What will your responsibilities be? Screening referrals for the Discharge to Assess pathway (7‑day rota; approx. 2/5 days per week once inducted) Post‑discharge assessments to deliver one‑off interventions, equipment, minor adaptations and manual handling recommendations Strength‑based reablement planning to support independence, recovery and prevent readmission Collaborative working with Reablement, Social Work teams, the Integrated Front Door and First Point of Contact services Hybrid working available after induction (home‑based documentation + community visits)
