About the role

Community School Coordinators are dedicated to leading school teams in the pursuit of education and care. The Community School Coordinators collaborate closely with members of the school leadership team and are responsible for the design, implementation, and oversight of a set of programmatic preventions, interventions and enrichment activities designed to support the holistic development of students and caregivers as determined by the annual Community School plan. Works closely with the Principal/Designee to co-design, develop and oversee all Community School programming at the school site. Leads a multi-disciplinary, multi-agency team to assess the culture and climate needs of the school community and creates a responsive, culturally relevant annual implementation plan. Ensures compliance with DMH billing requirements and activities provided, within CII's documentation standards and input of evaluation data used to measure ongoing program effectiveness of services and program outcome measures. Facilitates evaluation and program improvement gap analyses and needs assessment activities onsite frequently. Supports and participates in the Coordination of Services Team (COST) to oversee the linkage of students requiring support, timely implementation of interventions, the progress monitoring of students receiving interventions, and the evaluation of intervention effectiveness Assures the high-quality functioning of other Community School programming including health, dental, mental health, and dual generational support services. Collaborates and communicates with CII's FET and/or clinical team to ensure receiving and triaging of mental health referrals and/or urgent needs, as needed. Supports attendance initiatives, as indicated for school absenteeism and additional PBIS initiatives, as well as a multi-tiered intervention framework. Establishes strong, productive, and trusting relationships with the administrative team, school personnel, partnership organizations, students, and community to seek input on needs and address barriers to learning and social-emotional needs/ services. Manages day-to-day operations of the Community Schools model, including managing partnerships, site plans, and coordinating resources. Leads a committee of stakeholders (advisory councils) who meet a minimum of six times a year to provide input on the work. Communicates through use of social media, fliers, stakeholder presentations etc. to ensure that stakeholders are aware of the assorted opportunities. Forms partnerships with agencies in the community that enrich the school environment and will provide services for children and families: mental health, before, during and after school enrichment, summer camps, arts, sports, adult programs, medical, mentoring, as needed. Monitors Community School's budgets and collaborates with the partnership manager and if at a grant-funded school, collaborates with the school's operations and finance manager as well as grant accountants to review quarterly fiscal reports on funds spent and the impact of programs. Reviews and monitors progress on grant goals and metrics on a quarterly basis by analyzing data, impact, and providing recommendations on partnership expansion Collaborates with the grants team to ensure the timely submission of all required reports and supporting materials. Performs all other duties as necessary for the good of the agency as requested by the direct supervisor. ADDITIONAL DUTIES Other duties and special projects as assigned.
