Liquid Personnel is recruiting for an experienced Social Worker to join its client in a Learning Disabilities Team in Rhondda Cynon Taf. This role focuses on undertaking strengths-based assessments and reviews in line with the Social Services and Well-being (Wales) Act 2014, supporting adults to achieve independence, choice and positive outcomes. You will manage a caseload completing reviews for individuals on a waiting list, working collaboratively with families, carers and partner agencies to deliver high-quality, person-centred care. What will your responsibilities be? Undertaking assessments and reviews in line with the Social Services and Well-being (Wales) Act 2014 Managing a caseload within a Learning Disabilities service, completing timely reviews and reassessments Developing and updating person-centred care and support plans, ensuring outcomes are clearly identified and monitored Promoting a strengths-based approach, identifying and building on individual and community resources Assessing and managing risk, ensuring safeguarding responsibilities for vulnerable adults are met Supporting individuals, families and carers to maximise independence, choice and control Working collaboratively with health professionals, providers and other agencies using a multi-disciplinary approach Completing accurate, timely and high-quality case recordings, assessments and reports Participating in and contributing to multi-agency meetings, including safeguarding and planning meetings Making informed decisions regarding referrals, thresholds and urgency of response Providing information, advice and signposting, including access to advocacy services Supporting the commissioning and coordination of care packages, ensuring resources meet assessed needs Monitoring and reviewing care and support plans, making adjustments where required Ensuring compliance with departmental policies, procedures and statutory requirements Using supervision effectively to reflect on practice and manage workload Qualifications and Experience: Professional Qualification: MA, BA or BSc in Social Work, Diploma in Social Work, or CQSW Registration: Must be registered with Social Care Wales
