Tufts Medicine Care at Home is currently seeking a full time Nurse Practitioner to join our team in the greater Malden area. This position will be primarily geared towards home-based palliative care with occasional virtual hospice coverage. Our palliative care team includes physicians who are board certified in hospice and palliative care, nurse practitioners with extensive experience in pain and symptom management, and social workers who are knowledgeable about the physical, emotional and spiritual issues that often accompany a serious or life-limiting illness. How You'll Transform Patient Care: Remain current on clinical standards of practice in hospice and palliative care; promotes evidence-based practice Provide medical expertise on pain and symptom management Provide discussions on goals of care with all patients Identify/evaluate issues; reache sound conclusions; generate alternatives; make accurate and timely decisions Maintain documentation of patient visits, and communicate recommendations to colleagues Utilize interpersonal skills to establish and maintain relationships within the medical community
