Deliver specialised pharmacotherapy counselling with an emphasis on chronic disease management, medication safety, and enhancing patient quality of life. Review and verify prescriptions to ensure legal compliance, completeness, accuracy, and clinical suitability. Collaborate with prescribers to address medication-related concerns and escalate complex issues when appropriate. Oversee dispensing, labelling, repackaging, and compounding activities in accordance with regulatory and organisational requirements. Conduct medication reconciliation during care transitions, identify discrepancies, and ensure appropriate documentation and resolution. Detect, prevent, and manage medication-related problems through comprehensive medication reviews and optimisation of treatment plans. Educate and counsel patients, caregivers, and healthcare professionals on medication use, therapeutic outcomes, and safe medication practices. Support patient discharge processes through medication counselling, adherence support initiatives, and follow-up resolution of medication-related concerns. Contribute to inventory management activities, including stock monitoring, storage compliance, expiry control, and investigation of inventory variances. Assist with procurement functions by liaising with suppliers, coordinating purchases, and supporting cost-containment strategies. Utilise pharmacy and clinical information systems for documentation, reporting, data management, and issue resolution. Participate in quality assurance projects, clinical audits, service improvement initiatives, and research activities. Maintain compliance with all applicable legal, ethical, professional, and organisational requirements.
