Job Summary: The Service Coordinator fulfills all duties of the Designated Coordinator in a Rule 245D program licensed by the MN Department of Human Services. This position provides case management services, direct care and instruction for adults with developmental and other disabilities in a day training and habilitation program. Duties include: Working directly with persons in the program throughout the day and providing leadership/work direction to other program staff to ensure that individualized plans are properly implemented and that people served in the program receive quality services. Performing all aspects of internal case management, developing individualized program plans, acting as a member of the support team for persons served, acting as the liaison with each participant's family and residential provider. Planning and implementing individualized and small group activities within the center and the community. Working closely with the Program Supervisor and Program Director to ensure quality services and adherence to all program and regulatory standards. Hours, Benefits: 7:30 AM to 4:00 PM, Mon- Fri; full time. No evenings or weekends. Competitive wages. Must possess the education and experience necessary to provide quality services which would include: A BA/BS degree in a related field and 1 year work experience providing direct care for persons with disabilities; or an AA degree in field related to human services and 2 years of work experience providing direct care for persons with disabilities; or a diploma in a field related to human services from an accredited postsecondary institution and 3 years of full time work experience providing direct care services to persons with disabilities; or A minimum of 50 hours of education and training related to human services and 4 years of full time work experience providing direct care services to persons with disabilities and older under the supervision of a staff person who meets the qualification identified in the above criteria. You must also have a valid driver's license, a clean driving record and be insurable under the standards set by our insurance company. Candidates must pass a drug screen and criminal background screening as determined by DHS and MSS. Must be able to frequently stand, bend, stoop and reach and push manual wheelchairs used by adults. Note: The ideal candidate must be comfortable providing leadership and working in a team environment Preference will be given to candidates who have experience in working with people who have Autism, experience in art, theatre or music; and has experience with MN Statues 245D.
