Responsibilities

  • Clinical Excellence & Symptom Management
  • Conduct comprehensive home-based nursing assessments for palliative patients.

  • Participate in multidisciplinary case conferences to optimize patient outcomes.

Requirements

  • Qualifications: Registered Nurse with a valid Singapore Nursing Board (SNB) practicing certificate. A Diploma or Degree in Nursing is required.
  • Specialization: An Advanced Diploma or Graduate Certificate in Palliative Care or Gerontology is highly preferred.
  • Experience: Minimum 3 years of clinical nursing experience. Prior experience in Home Care, Community Nursing, or Oncology is a significant advantage.
  • Core Competencies: * Strong clinical judgment and the ability to work independently in a home setting.
  • Excellent communication skills to navigate sensitive end-of-life conversations.
