Learn more at shn.ca Job Description: The Pharmacist will be responsible for meeting the medication related needs of the patients which will include maintaining records and drug profiles, education to patients, families and caregivers and to positively contribute to maximize medication safety for all patients and the medication management system. Position Overview: Practice in accordance with the Ontario College of Pharmacists Provide navigation expertise for patients and staff regarding medications including; access, funding, best practice protocols and choosing wisely Pharmacist will work with an interprofessional group of health care providers and patients to provide patient-centered care that optimizes safe, effective, and appropriate drug therapy Assess, plan, implement, and evaluate the medication needs of patients in collaboration with the interprofessional team Key role in medication reconciliation Review of physician's orders, creating and maintaining computerized patient medication profiles, reviewing drug information and adverse drug reaction reporting Provide educational resources for staff and patients as required Experience with medications for patients of all ages with varying healthcare needs Demonstrated experience in family medicine/primary care settings Confer with various clinical resources, liaising between hospital, specialists, and community services Support evaluation and quality improvement activities to assess impact and outcomes and determine strengths, opportunities, and future directions of the IPCT services
