You'll be working with individuals aged 65+ with a range of needs, including physical frailty, dementia, and long-term conditions. What will your responsibilities be? Undertake strengths-based assessments and reviews Develop and review care and support plans Work closely with health professionals, families, and community services Apply the Care Act 2014, Mental Capacity Act, and safeguarding procedures Promote independence and person-centred outcomes Use Mosaic / Liquidlogic (or relevant case management system)
