POSITION SUMMARY The Nurse Practitioner (NP) at CHEO is a Registered Nurse with advanced education (Masters or Doctoral prepared), skills and experience to provide personalized and quality care to the communities that CHEO serves. NPs are regulated by the College of Nurses of Ontario (CNO) and hold a license as RN (Extended Class), with an independent scope of practice. As regulated health care professionals, the NP scope includes, but is not limited to, diagnosis, ordering and interpretation of diagnostic tests, prescribing medications, and performing medical procedures. Nurse Practitioners function as both autonomous and collaborating health care providers who integrates the knowledge and skill from both nursing and medicine disciplines. In addition to the clinical role, the NP embodies leadership, education, research and ongoing professional development to create a holistic perspective on the domains of advanced nursing practice. JOB DESCRIPTION Performs and documents comprehensive patient assessments based on thorough history taking from the patient and/or caregiver and physical examination Develops a plan of care in collaboration with the patient, family and interdisciplinary team as appropriate Orders and prioritizes diagnostic tests and initiates procedures as needed Interprets laboratory, radiology and other test results in relation to patient assessment and differential diagnoses Prescribes pharmacological and other therapeutic interventions Monitors treatment and modifies treatment plans accordingly while evaluating outcomes Communicates diagnoses, treatment plans, and disease specific education to patients and caregivers Collaborates with interdisciplinary team members to create holistic plans of care for patients Provides follow-up care for patients requiring regular visits Performs skills and procedures as per competency within clinical specialty Identifies patient and caregiver socioeconomic needs while making informed decisions about clinical care Consults and refers to specialist providers or health disciplines as needed for care delivery Documents all elements of patient work-up and care in the EMR during or following each patient encounter Provides transfer of care through consultation for patients who require alternative care that is beyond the knowledge and clinical skill of the NP Authorizes, facilitates, and coordinates admission, transfers, leaves of absence, and discharge of patients admitted to hospital Participates in care coordination through clinical case management via direct patient communication (telephone calls, virtual visits), patient conferences/multi-disciplinary meetings, rounds, clinic visits Participates in health promotion and disease prevention through verbal and written communication mechanisms (pamphlet development, program development, educational lectures) related to
