Requirements

• Experience in caring for patients with geriatric issues such as cognitive impairment/dementia, falls, polypharmacy, and goals of care
• Strong communication and interpersonal skills, with the ability to establish rapport and trust with older adults and their families.

Location: Rolland & Kathryn Lowe Medical Center Job Summary: Asian Health Services (AHS) is seeking a Provider to join our team at a Federally Qualified Health Center (FQHC) as part of our Senior Access and Geriatric Expansion (SAGE) clinic. The ideal candidate will be passionate about providing high-quality care to elderly populations, particularly those facing complex health challenges. You will have extended time during clinic to see your patients and will work within a multidisciplinary team to ensure that our senior patients receive comprehensive, culturally competent care aimed at improving their quality of life.
