Requirements

• Bachelor in Social Work or relevant fields (Psychology, Community Work, Healthcare);
• At least 2-3 years of relevant working experience; Knowledge and experience in eldercare/healthcare sector will be advantageous;
• High proficiency in written and spoken English;
• Strong interpersonal and communication abilities with individuals and systems at multi-levels;
• Good facilitation skills
• Team player with an ability to function effectively in a fast-paced environment.

Additional Information

KEY RESPONSIBILITITES Support care integration work in Toa Payoh subregion with partner organisations, including SCC, AAC, HPC+, and HT providers, and to achieve the deliverables specified for the Integrated Community Care Provider. Work with ground teams to implement the sub-regional workplan, and monitor and report on the progress of the workplan. Coordinate and facilitate regular meetings and check-ins for partner organisations to build understanding and work through implementation and other challenges. Engage and build networks and partnerships with other community care providers, regional health systems, grassroot organisations and other stakeholders. Assess resources and identify opportunities for care integration and coordination collaborations across ICCP partners and with external partners Support in the development and implementation of joint referral workflow with ICCP partners to support care assessment, planning and coordination for seniors. Review and track the effectiveness of interagency workflows in seniors' care.
