  • Per diem, varied days shifts as needed. No set guaranteed hours in a two week pay period. This is not a full or part time position.

  • The hours employees work determine when a shift differential is paid.
  • Hourly Evening Shift Differential: $5.41

Welcome to Montage Health's application process! Job Description: The Nurse Practitioner (NP) is an expert in diabetes care. The NP is responsible for the delivery of diabetes care and education that involves compliance with evidence-based clinical standards, outcomes measurement (tracking, analysis, and reporting of clinical, financial, and satisfaction outcomes), and process design (leading initiatives to facilitate practice-based and organizational change). As a member of the glycemic management team, the NP provides direct patient care in the outpatient and community setting through implementation of standardized procedures, collaboration with the medical team to establish priorities, and assessment of solutions for patients to receive exceptional care. The NP functions under general Nurse Practitioner Standardized Procedures approved by the Committee on Advanced Practice Providers (CAPP) and reports to the Diabetes Services Program Manager.
