A Day in the Life Working in partnership with other case managers, physicians and patients, you will identify and assist patients with complex post-acute HIV needs and facilitate the development of a safe discharge plans. You will be responsible for assessing the patient's continuum of care needs, developing a plan in collaboration with the patient the healthcare team, and implementing the plan using the resources of the Resource Center. You will advocate for the patient and family throughout the entire process. What's Required Current Michigan licensure as RN, LLBSW, or LBSW required, or eligible for licensure within a timeframe determined by Human Resources. Experience working with persons living with HIV and/or in medical case management preferred but not required. Certification as an HIV Case Manager and HIV Testing/Counseling through the State of Michigan to be completed within the first year of employment. Possesses a valid driver's license and is eligible for Class III Chauffeur license. Demonstrated oral and written communication skills and competency in Microsoft Office products and electronic medical record systems. Are you Munson Material? Apply today!
