About the role

The EMR Optimization Lead will serve as a foundational leader at Medical Home Network (MHN). The EMR Optimization Lead is responsible for building and scaling our EMR optimization function to support value-based care across our network of partner organizations. This individual will play a critical role in advancing MHN's mission to transform care in the safety net, improve health outcomes, and reduce disparities in underserved communities. As the first hire in this function, this leader will not only drive EMR optimization strategy and execution but will also establish best practices, build strong partner relationships, and lay the groundwork for a high-performing team. This role partners closely with community health centers, provider groups, and internal MHN teams to optimize EMR systems, enhance clinical workflows, and enable actionable, population-level insights that support value-based care initiatives.
