About the role

Complete assessments, reviews, and care planning within the community Support individuals to maximise independence using strengths-based approaches Assess for equipment, adaptations, and moving & handling needs Work collaboratively within multidisciplinary locality teams Manage a varied caseload in a fast-paced environment

Requirements

• HCPC-registered Occupational Therapist
• Experience in Adult Social Care / community settings
• Strong assessment and clinical reasoning skills
• Ability to prioritise and manage workload effectively
