Emplify Health by Gundersen is seeking a Pharmacy Medication History Assistant to join our hospital pharmacy team in La Crosse, WI. This role is a great opportunity for students enrolled in a healthcare pre-professional program, such as pre-pharmacy, who are interested in gaining hands-on hospital pharmacy experience. Under the supervision of a pharmacist, the Pharmacy Medication History Assistant helps complete accurate medication histories for patients admitted to the hospital. Responsibilities include interviewing patients, families, and caregivers, verifying medication information with outside sources when needed, and entering reconciled medication lists into the electronic health record (EHR). This position functions as a pharmacist extender and supports safe, accurate medication reconciliation in the inpatient setting.
