Medical Concierge, Perennial Living

Responsibilities

• Monitor residents' health status through regular observation and routine vital signs monitoring.
• Promptly escalate and communicate any observed changes in residents' health condition tothe nursing team or attending medical professionals.
• Support and coordinate clients' healthcare journeys as a trusted healthcare navigator.
• Arrange and coordinate escort services for residents' medical appointments,investigations and hospital visits.
• Track and follow up on healthcare appointments, referrals and recommended reviews to ensure continuity of care.
• Serve as a liaison between residents, families and healthcare providers to facilitate smooth coordination of care.
• Monitor care progress and ensure timely follow-ups and updates to residents and/or families
• Collaborate closely with the Personal Concierge to develop and coordinate residents' careplans.
• Maintain accurate records of residents' health monitoring, appointments and follow-upactivities.
• Any other duties assigned.

Requirements

• Diploma or Degree in Healthcare Administration, Hospitality, Business, or related discipline.
• Candidates with client-facing experience in private healthcare, hospitality, concierge services will have added advantage.
• Highly organised and dynamic.
• Excellent interpersonal and communication skills.
