Responsibilities

  • Supporting individuals before release, through the gate, and into the community
  • Building strong relationships with prison substance misuse teams and partner agencies
  • Providing enhanced support to individuals at risk of disengaging from services
  • Accompanying clients to key appointments including probation and substance misuse services
  • Identifying and managing risks around release, including treatment drop-out and overdose prevention
  • Completing assessments, action plans, and maintaining accurate case records
  • Motivating and empowering people to engage with statutory and voluntary support services
