Ennoble Care is seeking a full-time Care Coordinator, Embedded @ Partnered-Dialysis Clinics in the Tulsa, Oklahoma area. This on-site role serves as a key liaison within a partnered dialysis facility, ensuring coordinated, patient-centered care through structured coordination, strong clinical collaboration, and proactive communication. This Care Coordinator works directly within one of our partnered-dialysis clinics to engage patients in person, coordinate care plans in collaboration with Ennoble providers and facility staff, schedule and organize provider visits, and ensure timely follow-up and clear communication with patients, families, and the clinical team. This position plays a central role in maintaining continuity of care by monitoring patient needs, communicating changes in condition, addressing barriers to care, and maintaining accurate documentation to keep the care team aligned. Responsibilities

  • Provide consistent, on-site engagement with dialysis patients to support care plans, follow-ups, education, and care transitions
  • Coordinate and align patient care using Ennoble Care's EMR to ensure continuity, quality outcomes, and timely follow-up
  • Schedule patient visits for providers and support daily clinic workflow based on patient attendance and clinical priorities
  • Collaborate closely with Ennoble Care providers, dialysis facility staff, and a multidisciplinary care team to support integrated, patient-centered care
  • Serve as a liaison between providers, patients, families, and facility staff, ensuring clear, proactive communication
  • Communicate patient needs, changes in condition, safety concerns, and urgent matters promptly to the appropriate clinical leader
  • Identify and resolve barriers impacting patient care coordination and overall experience
  • Maintain accurate, timely documentation in accordance with organizational standards, workflows, and healthcare regulations
  • Demonstrate professionalism, empathy, and sound judgment while effectively managing multiple priorities in a fast-paced clinical environment
  • Contribute positively to team collaboration, daily operations, and facility partnerships while maintaining strong organizational and technical proficiency (including Microsoft Word and Excel)

Requirements

  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure required, such as:
  • Patient Care Technician (PCT)
  • Certified Nursing Assistant (CNA)
  • Medical Assistant (MA/CMA)
  • Licensed Practical Nurse (LPN)
  • Registered Nurse (RN)
  • Or comparable clinical credential
  • Highly preferred: Dialysis-specific certification (CDT, CHT, CCHT, CCHT-A)
  • Highly preferred: Experience managing provider schedules and coordinating patient appointments within a clinical setting
  • Highly preferred: Minimum one (1) year of experience working directly with hemodialysis/nephrology patients in a dialysis facility
  • Demonstrates a professional, compassionate bedside manner with a patient-centered approach and the ability to build rapport in a clinical setting
  • Strong working knowledge of healthcare terminology and care coordination principles
  • Excellent organizational and time-management skills with the ability to manage multiple priorities effectively
  • Professional and proactive communication skills across Microsoft Teams, email, phone, and in-person interactions
  • Ability to cultivate and maintain positive, collaborative relationships with patients, families, providers, and facility staff
  • Proficiency in Microsoft 365, including Outlook, Word, and Excel
  • Ability to work full-time, Monday through Friday, 5:00 a.m. to 1:00 p.m. CST in an on-site role based in the Tulsa, Oklahoma area
  • No weekends, evenings, nights, or on-call requirements
