Complete Care Act assessments and reviews for adults with learning disabilities, ensuring support plans promote independence, choice, and control. Work collaboratively with health professionals, providers, and families to coordinate person‑centred support and manage complex needs. Safeguard adults at risk, carrying out risk assessments and contributing to multi‑agency planning to ensure safety and wellbeing.
