The purpose of this position is to have a Care Coordinator that is responsible to assist the patient, their families and provider across all aspects of their health care continuum. The care Coordinator is the liaison between the patient and the provider with an optimum goal of meeting the patient's individualized needs, ultimately improving quality of life which will result in positive patient outcomes through communication, education and coordination. Minimum Education: REQUIRED: - High School diploma or equivalent - Graduate of an accredited License Practice Nurse program or Medical Assistant Diploma/Certificate from an accredited program. Minimum Experience/Knowledge: - One-year experience in the medical field (preferably as a MA or LPN in a clinic setting). Required License/Certification: REQUIRED: - BLS for Healthcare Provider from the American Heart Association If LPN or MA, must have following: - If License Practice Nurse: Current and Unrestricted LPN NV License - If Medical Assistant: Medical Assistant Diploma/Certificate from an accredited program. - Fire and Safety Certification. If no card upon hire, one must be obtained within 30 days of hire, and maintained by renewal before expiration date. (Required within LA City only) USC is an equal opportunity employer.
