- Core Purpose:
- To provide specialised student support through supportive guidance, programme management, and collaborative intervention strategies to help Students with additional needs (SWANS) meaningfully connect with their school community and achieve personal growth.
- Expected outcomes include improved school engagement, enhanced social-emotional skills, and stronger peer relationships.
- Programme Management and Implementation
- a. Work with vendors to customise, and implement structured support UPLIFT programmes (eg. LOL and GLOW programmes) tailored to meet diverse student needs.
- b. Create and curate programme resources whilst managing administrative tasks including student progress monitoring
- c. Conduct twice-weekly student check-ins during the planned UPLIFT programme/activities with follow-up support for students requiring additional assistance.
- (2) Direct Student Support
- a. Provide individual and group support to students across all year levels facing academic, social, or emotional challenges (based on identified student list provided by the school)
- b. Build positive relationships and implement early intervention strategies
- c. Conduct relevant workshops for students and staff (eg. conflict resolution clinics) to work towards desired student goals.
- (3) Collaboration and Communication
- a. Work collaboratively with Year Heads, School Counsellors, Form Teachers (FT), and FLY/GLOW/UPLIFT programme providers to monitor students' progress against the identified learning outcomes
- b. Maintain regular communication with parents and caregivers (based on identified student list provided by the school)
- c. Provide comprehensive reporting on student progress - regularly with Form teachers, termly for SDT KPs and semesterly with School Leaders.
