About the role

The primary purpose of the social work role within the Hospital setting within the Adult Social care department is to ensure that vulnerable service users admitted into various acute and secondary hospital setting are discharged safely within the community with a focus on person centred holistic assessments to reflect the goals and outcomes that service users want to achieve. What will your responsibilities be? In this role, you will conduct Psychosocial Assessments, evaluate patients' social, emotional, and environmental needs to develop appropriate care plans.
