Department: Mobile Clinic The Community Health Advocate (CHA) is responsible for supporting community members and their social and health needs by referring them to community resources, services and medical systems. The Community Health Advocate will be based in the SVH Mobile Clinic as part of the care team. Actively engages, builds rapport, establishes a trusting relationship and candid communication with community members, member family support systems when available and practice providers and staff. Assesses social determinants of health needs for moderate-risk community members and documents assessment results or augments available information in electronic medical record. Consults with or refers community members to licensed staff (nurse practitioner, physician assistant, physician) as required based on community member social, health risk and medical complexity. Establishes relationships and partners with community resources, health plans and providers by participating in local community engagement activities with local agencies e.g. faith-based organizations, community centers, government agencies, parks, recreation centers and schools. Assists community members with problem-solving barriers to moderate health conditions by identifying, locating, referring to needed community and medical system services including assisting members with completing forms to access needed services. Determines social determinants of health concerns/ gaps, develops and documents a plan to address moderate social and health disparities. Greet patients at the Mobile Clinic and provide a friendly and welcoming environment. Identifies gaps in community resources and medical systems and supports the implementation of new solutions or services to close identified gaps. Responsible for data collection and reporting. Makes recommendations for program improvements. Actively participates in team meetings. Serve as professional representative of SVH. Adhere to all HIPPA regulation. Perform other duties as assigned Education: High school Diploma or GED required. Bachelor's degree in Social Work, Psychology, Biology, Public Health, Nursing, Community health or Health-allied field preferred. Licensure: Valid CA Driver's License Current BLS/Healthcare Provider status as per American Heart Association standards required. New hires/transfers have six (6) months to obtain. Language: Bilingual - English/Spanish required. Essential Technical/Motor Skills: This position requires physical and communication skills. Interpersonal Skills: Excellent time management and organizational skills Excellent written and verbal communication skills, must be able to communicate clearly and concisely with all levels of individuals, sometimes in stressful situations Ability to tailor information at the appropriate level of health literacy and to interact respectfully with diverse cultural and socio-economic populations Must be able to work flexible hours including evenings and weekends as needed Must be able to manage conflict, stress and multiple simultaneous work demands in an effective professional manner and be comfortable in an informal, friendly environment Must demonstrate respect, concern and empathy for the spiritual and emotional needs of patients Must be able to effectively work with patients, health service providers, agencies and others from diverse backgrounds to reduce cultural and social-economic barriers Excellent rapport and trust building skills with a disenfranchised population Knowledge of managed care concepts, health promotion strategies, behavior change techniques, and disease management and prevention protocols Ability to work independently, problem solve, and design creative solutions to complex challenges Detail oriented, organized, and efficient Ability to effectively engage with clinical partners and other stakeholders Strong commitment to teamwork with ability to contribute expertise and follow leadership directives Ability to thrive in a continually evolving environment with competing priorities Strong computer skills with proficiency in Microsoft Office (Excel), Outlook, Word, PowerPoint, electronic medical record systems (eMDs and Epic preferred), and web-based solutions Pay Range: The hourly rate for this position is $38.17 - $47.71. The range displayed on this job posting reflects the target for new hire salaries for this position.
